Fix missing plugin names in log, filter quirk 
- Re-add the [PluginName] tag to plugin log messages
- Remove source prepending from the SerilogEventSink
- Force a filter refresh on enabling/disabling log filtering
- Convert ModuleLog to properly tag its messages with SourceContext
